Diagnostic Referral Consent Message

 

Following your recent Doctor Care Anywhere appointment, a referral is being made for you to have some diagnostic tests and/or treatments with a specialist.

 

Before you can see the specialist, your insurer will need to approve the referral so that they can process your claim. If you like, we can send the referral approval request direct to your insurer on your behalf. However, before we can share your details, we will need your consent to do so.

 

Who will you be sharing my details with?

In order to allow your journey to be as seamless as possible, we will need to share your information with AXA Health, clinical partners** and our specialist consultants. Information will need to travel back-and-forth between each of these groups.

Please be assured that we will not share any information without specific purpose. Medical information will need to be shared to enable consultants to have everything necessary to make their recommendation. Your personal identity information will also need to be shared to protect your privacy and to make sure that your medical journey is as streamlined as it should be.

 

What details of mine will you be sharing?

To enable AXA Health to assess the eligibility of your claim, administer your account and pay any claims, they need to know some of your personal information and relevant medical history. You can see the exact content that we will share with them by reading the referral approval request, however in summary these are:

1. Your full name;
2. Your date of birth
3. Your contact details
4. Your AXA Health membership details
5. The medical information that is relevant to this referral
6. Any diagnostic Investigations that the doctor has requested
7. The type of specialist that the doctor is referring you to; and
8. Any specific communication that the doctor wishes to relay to AXA Health.

We will continue to discuss your progress with AXA Health for the duration of your care, but we will not share any further medical information with them without your consent.

Your full patient record will be made available to our specialist consultants and clinical partners to enable them to provide you with high quality care.

 

What do I need to do?

Your referral approval request and referral letter are ready for you to read right now. To allow us consent to share your information, accordingly, simply follow the steps below:
1. Firstly, you will need to log in to your Doctor Care Anywhere account
2. Click on the relevant appointment in your list of 'Past Appointments'.
3. A PDF of your referral approval request and your referral letter is available for you to read.
4. Select 'Yes' if you are happy for us to share your personal information as described above; or 'No' if you aren't.

 

When the referral is approved, what happens next?

If the claim is approved, you’ll be contacted to arrange the diagnostic tests. After all your test results are available, we’ll then arrange for two of our specialist consultants to review your medical notes as well as the test results. Our consultants will recommend a management plan and advise whether further specialist input will be needed. This will all be discussed with you at your follow-up appointment with a Doctor Care Anywhere GP and if you need to see a specialist in person, this will be arranged for you.

 

What if I don’t want to give consent or I change my mind?

If you would prefer for us to not share your personal information, you can select 'No' in answer to the consent question.

Being able to share your personal information allows Doctor Care Anywhere to make your journey as seamless as possible. Without your consent, we will not be able to facilitate your journey into specialist care, but you can still contact AXA Health directly to discuss your options. They can be contacted on 0800 0281503.

You can change your mind at any time by getting in touch. From that point we will stop sharing your personal information and, as a result, will mean that we will no longer be able to continue your specialist care with us.

However, this does not impact our ability to support you with any existing or future healthcare needs; our GPs are only an appointment away.
